Hispanos Unidos provides medical case management services at both its New Haven and Meriden sites. The goal of medical case management is to ensure optimal health outcomes. Medical case managers conduct ongoing assessment and evaluation of clients’ physical, emotional, and psychosocial needs and provide linkage to appropriate medical and support services. Medical case managers assist clients in identifying resources and services that might assist them in meeting their basic living needs. In addition, the medical case managers may also provide emergency financial assistance for help with HIV-related medical bills and living expenses to those individuals who meet the eligibility criteria.
